Defining patients at-risk of non-response to cardiac resynchronization therapy. Value of rest and exercise echocardiography

Mechanical dyssynchrony and its potential role in cardiac resynchronization therapy (CRT) have been extensively investigated over the past 10 years. There are arguments to believe that echocardiography may prove useful. It has been suggested that an integrative, multi-parametric approach could be of clinical value. Data from recent studies using the speckle tracking technology rather than the less reliable tissue Doppler approach provided encouraging results. The aim of the present study was to assess the value of rest and sub-maximal (maximal heart rate = 100–120 beat/min) exercise echocardiography with a special focus on mechanical dyssynchrony and cardiac reserve, to predict the 6-month echocardiographic response to CRT.
